AM homes is a home and community based service provider that works to improve lives by providing individualized services and support to people in their homes or in our DHS approved units. Our person-centered service coordination model provides the tools needed to promote independence, self sufficiency, quality of life and optimal health. We are community partners that nurture relationships with families, communities, providers and related systems.
The services we provide vary from behavior supports, daily living assistance such as cooking and cleaning to more esoteric topics such as social skills, communication or community access. No matter the need or concern for our clients and their families we aim to address those needs directly.

OUR SERVICES
INTERGRATED COMMUNITY SUPPORTS (ICS)
Designed for adults to receive onsite planned and unplanned support when living in multi-family housing (e.g. apartment). ICS can be delivered up to 24 hours per day in the person’s living unit or in the community.
The service provides support and training in multiple areas to community living including, but not limited to:
- Community participation
- Health, safety and wellness
- Household management
- Adaptive skills
Our staff meets with families, interdisciplinary team members and the client regularly to monitor expectations. The schedule design is flexible to meet the client’s needs. Eligible Waiver: BI, CADI
NIGHT SUPERVISION
Night Supervision is a one-to-one service that takes place in a person’s home. Staff assist with following a person’s plan, encouraging skill development, and helping with activities of daily living (ADLs and IADLs). Family and friends can be hired as support staff, providing relief for primary caregivers. Night Supervision can be used for up to 12 hours in a 24 hour period. Eligible waiver: CADI, CAC, BI, DD
